Job description

We have a unique opportunity for a dynamic and highly organised individual to join our Trauma & Orthopaedics (T&O) team as a Patient Pathway Coordinator. This is a key role within a busy clinical service, ideal for someone who is passionate about delivering high standards of patient care and administrative excellence.

As a Patient Pathway Coordinator, you will be the main point of contact for patients accessing elective hospital services, guiding them seamlessly through each step of their care journey. You’ll play a vital role in ensuring that patients feel supported, informed, and valued at every stage — from the moment of referral to their final discharge.

Working under the direction of the Operations Management Team and closely with Consultants and clinical staff, you will take ownership of the coordination and management of patient pathways, ensuring they are delivered smoothly, efficiently, and within agreed timeframes. Your role will be instrumental in helping the team meet performance targets and improve patient outcomes.
