Job description

Due to maternity leave cover, we have an opportunity to offer a fixed-term position within our patient flow team to a dynamic and resourceful Nurse, Occupational Therapist, or Physiotherapist. We are a small but essential team working across Oxfordshire Community Hospitals to ensure patients' journeys are as efficient as possible. Our team collaborates closely with system partners in Adult Social Care and Oxford University Hospitals, providing an opportunity to understand the wider health and social care system.

We support secondments with your line manager’s agreement and consider flexible working requests. Our work spans all community hospital sites, includes access visits to patients' homes, and in-reaching into acute hospital sites as needed.

As Part Of Our Team You Will
  1. Support our discharge coordinators by providing expertise to all wards.
  2. Contribute to decisions regarding patient pathways at Transfer of Care Hub meetings.
  3. Collaborate closely with ward multidisciplinary teams to understand patients’ support needs upon discharge.
  4. Work with patients and families to overcome challenges in discharges from hospital.
