The NHS seeks a Patient Flow Co-ordinator for a new integrated mental health service supporting youth in Sussex. The role involves managing patient data, facilitating treatment pathways, and ensuring efficient communication between services for children and young people facing acute mental health challenges.
Job summary
We are seeking to recruit an experienced and efficient Co-ordinator to join an exciting new development of an enhanced crisis / intensive home treatment 7 day a week service. This is a brand new integral role co-ordinating the front door, referrals, patient information and data and working to the service performance timeframes.
You will be part of an exciting development to give children and young people living in Sussex a more appropriate integrated range of services to meet their clinical needs. As the Patient Flow Co-ordinator you will work in partnership with the clinical team and Provider Collaborative to assist young people accessing acute care at the right time and in the right place. This will involve overseeing young people in an inpatient bed and also being seen in the community including intensive home treatment.
The aim of the Sussex CAMHS 'hospital at home' is to provide an alternative to admission to support the local network around the young person (aged 12-18) whilst they are in the acute stage of mental health difficulties. You will be responsible for managing data, patient information and overseeing the patient journey through acute care via data management and other essential administrative processes.
The Getting it Right First Time (GIRFT-2022) report highlights the 'importance of ensuring young people receive the right treatment close to home and make sure that their family, social and educational networks do not break down'.

o To promote and demonstrate the ability of Inpatient & intensive home treatment services working together to deliver care that is efficient and at the right place at the right time.oTo offer young people and families a seamless acute journey which keeps them well informed, ensures they feel supported and provides them with the right care and treatment at the right time.oTo enable young people & families to access an inpatient/alternative to inpatient service offer which avoids delay and is not stifled by barriers created by individual services.oTo develop effective working relationship with local teams and services to support and optimise patient flowoParticipate as a core member of CAMHS Acute leading on patient flow Monday-Sunday 9am-8pm shift pattern with the expectation to support colleagues annual leave
