Job summary

Carers Leeds is seeking a Patient Flow Carer Support Worker to work with hospital discharge teams and carers to arrange permanent placements or domiciliary care for self-funding clients.

The role is 35 hours per week, fixed term until 30 June 2029, with a mix of office, community locations and home-based work across Leeds.

Qualifications

  • DBS check required.
  • Experience in carer support or hospital discharge planning is beneficial.

Responsibilities

  • Develop close working relationships with Community Care Bed MDTs across the city.

Key duties and responsibilities
  • To develop a close working relationship with the Community Care Bed Multi-Disciplinary Teams across the city.
  • To provide person-centred support, advice and information to carers, and those with care needs, to support discharge to permanent care (or home with a package of domiciliary care) where the person with care needs will be self-funding this care.
  • To undertake monitoring and evaluation of the service and prepare regular reports on the work undertaken.
