Responsibilities

• Develop close working relationships with Community Care Bed Multi-Disciplinary Teams across the city.
• Provide person-centred support, advice and information to carers to support discharge to permanent care or home with domiciliary care, including self-funding arrangements.
• Monitor and evaluate service delivery and prepare regular reports on work undertaken.
