Job description

An exciting opportunity has arisen within the Patient Flow Team for a full-time Band 3 who has an interest in the flow of patients within a busy acute NHS Trust.

AMS is a large CSU within St James Hospital comprising a dedicated surgical assessment unit and 11 inpatient wards. We are a regional centre for urology, colorectal, gastroenterology, upper GI, HPB, and renal services, as well as being responsible for the Leeds Liver Unit, one of a handful across the country.

Main duties of the job

The post holder will ensure the safe and effective utilization of hospital capacity and patient flow, undertaken under the supervision of a registered practitioner. This includes scheduled and unscheduled admissions, internal and external transfers of care.

The post holder will liaise with staff and wards to ensure effective, efficient patient flow, gather data for reports, and assist the Nurse in Charge in decision-making regarding patient movement.

Establish accurate, reliable information to allocate and utilize capacity corporately, balancing conflicting pressures.

About us

You will join an experienced team managing all acute and elective patient flow throughout the AMS CSU, which includes 11 inpatient wards, endoscopy, and a dedicated assessment unit. This role is vital to the safe and effective delivery of care in our specialties, including gastroenterology, urology, hepatology, upper GI, renal, colorectal, and surgery.

Many AMS patients are assessed and admitted via the Surgical Assessment Unit, requiring close collaboration with JSAU.

Job responsibilities

Collect, collate, and report capacity and patient flow data to the Nurse in Charge.

Identify and record planned/elective admissions daily, liaising with relevant staff to plan bed allocations under supervision.

Coordinate with staff across wards to establish bed counts and availability.

Produce reports and updates on bed availability for the clinical site manager.

Support ward staff to maximize timely patient discharges.

Assess operational situations critically and solve problems with appropriate solutions.

Be visible and approachable across hospital areas to foster good relationships.

Engage with Surgical Assessment staff to reduce breaches and escalate concerns appropriately.

Maintain confidentiality at all times.

Communicate effectively with staff and patients via phone, email, and face-to-face.

Act as a role model for clinical staff on ward environments, promoting learning and development of patient flow.

Participate in audit activities and continuous personal development.

Seek opportunities for service improvement and contribute to a positive team environment.

Qualifications
  • Literate and numerate – Basic Skills Maths and English Level 1 (GCSE A-G) or equivalent experience within a Band 2 CSW post.
