Overview

Are you looking for a challenge which requires flexibility, forward thinking and drive? An exciting opportunity has arisen for a motivated enthusiastic individual to join the team on Chestnut ward our Medical Admissions Unit as a Patient Discharge Navigator. This role will help ensure a smooth and safe transition by coordinating discharge plans, accessing necessary resources, and providing information and support to our patients, their families and carers.

Responsibilities

• Work as part of the multidisciplinary ward team, gaining an insight and knowledge onto the needs of all the patients on their allocated wards.
• Support patients, their families and carers to ensure that assessments and discharge plans are fully understood and agreed.
• Attend the daily board meetings to proactively identify patients who are ready for discharge and transfer to a backend medical bed.
