An exciting opportunity has arisen for a motivated enthusiastic individual to join the team on Chestnut ward our Medical Admissions Unit as a Patient Discharge Navigator. This role will help ensure a smooth and safe transition by coordinating discharge plans, accessing necessary resources, and providing information and support to our patients, their families and carers.
You will need to work as part of a wider team but also be able to work autonomously, prioritising your workload and communicating delays and concerns in a timely manner.
