Job Summary

Hospice at Home, an independent charitable organization, is looking to recruit a flexible part-time Patient Coordinator and admin support worker. The post holder will work flexibly to meet the needs of the Nursing Coordination Team.

Main Duties of the Job

The post holder will be responsible for accepting referrals to the nursing service, allocating patient care, and providing general admin support to the clinical teams. The role involves daily communication with a variety of health and social care professionals, patients, carers, and the general public.

Hospice at Home's core service is delivered by a skilled team of registered nurses and healthcare assistants providing exceptional end-of-life care and support. Additional services include lymphoedema, bereavement support, family support, befriending, occupational therapy, and complementary therapy. Job Description

The post holder will act as a patient coordinator for the Hospice at Home nursing and support service, providing palliative care to patients, their relatives, and carers. Responsibilities include accepting referrals, staff allocation, liaising with health and social care professionals, and providing administrative support to various teams.

Responsibilities

Coordinate patient care, liaise with professionals, and support the Hospice at Home team including Nurse Team Lead, Complementary Therapy, Family Support, Lymphoedema, and Occupational Therapy services.

  • Good understanding of NHS IT systems
  • Ability to prioritize workload
  • Ability to work independently and in a team
  • Excellent communication skills
  • Ability to recognize distress in patients and families
  • Ability to handle distressed individuals
  • Concentration amidst changing priorities
  • Advanced IT skills

Desirable Skills

  • Knowledge of EMIS including reporting

Qualifications (Essential)

  • Business Administration qualification or equivalent

Desirable Qualifications

  • ECDL

Experience (Essential)

  • Experience in health/social care settings
  • Experience dealing with the public

Desirable Experience

  • Experience in palliative care
