A healthcare service provider in Bristol is seeking a Patient Coordinator to connect patients with necessary services, manage referrals, and schedule appointments. The ideal candidate will have experience in healthcare, possess excellent attention to detail, and demonstrate effective communication skills. We're an independent provider of health services, based inBristol and providing services across the South West. Our aim is to providecare closer to home in a timely way.
As well as our NHS clinics, we also provide a self-payingMinor Surgery service, and this is where we are looking to appoint anotherPatient Coordinator. Although the focus of the role will be enabling patientsto access our Minor Surgery service by matching their needs to an appropriatesurgeon, you will also provide cross cover support to our NHS services.

Working hours: up to 30 hours per week over 4 days, mustinclude a Friday.

You will work in the centralised clinical administration services basedout of Aztec West where the ability to use your own initiative, work in anempowered manner, multitask and prioritise activity are all key. In your role as Patient Coordinator you willbe the initial point of contact, mostly by phone, for patients referred bytheir GP practices into GP Cares services.
You will be responsible for receiving patient referrals and enquiriesonline, via email or by telephone and managing the patient documentationthrough triage to the clinic. You willmanage clinic schedules, booking patients into the appropriate clinics fordiagnostic tests, assessment and treatment while ensuring that clinic time ismanaged efficiently. Excellent attention to detail is required and maintainingconfidentiality in the management of critical patient information.

Enterpatient information accurately onto the appropriate patient management system,updating patient information where appropriate e.g. EMIS
