Main duties of the job
An exciting opportunity has arisen for a Care Coordinator to join our already established, growing, and committed PCN team to focus on Cancer and Learning Disability Patients, support DOAC monitoring and offer support to the Health and Well Being Coaches when required.

The successful candidate will possess strong organisational, administrative and communication skills with the ability to multi-task. You will need to demonstrate that you have a compassionate nature with drive for achieving high quality, safe care. You must be a conscientious, self-motivated individual with a methodical manner and the ability to work well on their own as well as part of a multi-disciplinary team.
The successful candidate will be joining a thriving existing PCN team of multi skilled staff and be an integral part of the team when it comes to delivering the best patient care.
