Main duties of the job

An exciting opportunity has arisen for a Care Coordinator to join our already established, growing, and committed PCN team to focus on Cancer and Learning Disability Patients, support DOAC monitoring and offer support to the Health and Well Being Coaches when required.

The successful candidate will possess strong organisational, administrative and communication skills with the ability to multi-task. You will need to demonstrate that you have a compassionate nature with drive for achieving high quality, safe care. You must be a conscientious, self-motivated individual with a methodical manner and the ability to work well on their own as well as part of a multi-disciplinary team.

The successful candidate will be joining a thriving existing PCN team of multi skilled staff and be an integral part of the team when it comes to delivering the best patient care.

Job responsibilities

Primary Duties and Areas of Responsibility

  • Supporting Learning Disability patients, maintaining contact and inviting them for their annual review.
  • Manage cancer review recalls and deal with invites for reviews for 3-month discussions and 12-month reviews
  • Search for and contact patients who did not attend bowel screening
  • Search for and contact patients who did not attend Breast screening
  • Invite newly diagnosed patients for initial review
  • Support new cancer project look at improving patient education in the younger age group 35-55 and creating an awareness of when to seek help.
  • Support the Health and Well Being Coach Team inviting patients to group sessions
  • Supporting the pharmacy technicians with contacting patients on blood thinning medication

Qualifications
