Job Description:

Purpose of Job:

Support optimal patient care by maintaining and documenting quality care measures, reporting findings to monitor statistical outliers, and managing information efficiently to improve patient and population health. Respond to oversight agencies' demands while maximizing incentives and minimizing penalties by meeting their goals.

Essential Functions:

  1. Implement and maintain quality standards necessary for primary care recognition.
  2. Identify, educate, support, and coach team members on documentation processes related to recognition standards.
  3. Coordinate transitions of care through communication with patients, caregivers, clinicians, and facilities according to policies and treatment plans.
  4. Schedule appointments and identify care gaps using guidelines; support patient self-management of chronic illnesses as per physician plans.
  5. Facilitate team meetings to discuss high-risk patients and develop initiatives to improve patient care quality.
Job Qualifications:

Education: High school diploma, medical assistant diploma from an accredited institution, or community college course in medical assisting. Current medical assistant certification preferred.

Experience: Minimum 3 years recent experience in a medical facility as a medical assistant preferred. Primary care office experience preferred.

Other Skills: Proficiency in Microsoft Office; current health records with necessary immunizations (hepatitis B, tuberculosis).

Knowledge: Understanding of healthcare protocols, medical office procedures, and patient communication.

Skills: Tactful interpersonal skills, effective patient education, and information sharing abilities.

Abilities: Ability to learn patient care procedures, maintain professionalism, prioritize tasks, and demonstrate compassion.
