A healthcare alliance in Castle Point is seeking a part-time Patient Care Coordinator. You will facilitate patient bookings, manage communication within the clinic, and provide administrative support, ensuring high-quality service delivery to enhance patient care in the community.
Job summary
GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is working in partnership with the local Primary Care Networks to support the delivery of the NHS England, Direct Enhanced Service (DES) contract.
We are looking to recruit a Patient Care Coordinator to join a wider team covering Canvey Island. This role will be based at the PCN Office in Canvey Primary Care Centre, Long Road Canvey Island Essex SS8 0JA.
You will be working Monday, Tuesday and Thursday 9am-5pm
Main duties of the jobSupporting the PCN clinics with administrative duties such as patient bookings and enquiries.
Producing data reports and audits from SystmOne.
Acting as the first point of contact for the clinical team, patients and visitors to provide support with general queries.
Maintaining the staffs clinical rotas on SystmOne.
Providing administration support to the PCN educational events.
Liaising with patients and members of the community.
This role involves providing a high quality, comprehensive and effective administration skills.
The appropriate individual will have SystmOne and Primary Care experience.
You will need to be confident individual who can communicate with a ride range of professionals at all levels.
The successful applicant will have the ability to work without supervision using their own initiative, organising and prioritising own and other workloads to changing and often tight deadlines.

The post holder is a member of the Primary Care Network (PCN) providing administrative support to the PCN clinics and staff. Scheduling sessions, updating patient records, and ensuring that the actions are completed by the relevant clinician. Being the first point of contact for patients and visitors. Proactively case finding patients for sessions using the GP Clinical System (SystmOne) as well as running reports. Booking the patients into the relevant clinics and keeping waiting lists up to date.
The post holder will also as be required to provide data collection to help the wider PCN team achieve other key quality improvement, patient safety initiatives and work programmes.
Job Responsibilities:
Utilising SystmOne to set up searches, data collection and creating rotas for the clinical team.
Providing rota administration for the clinical team on the PCN Clinical SystmOne. Including, generating new rota templates, making necessary adjustments and cancelling as appropriate.
Explain the management of a patients pathway to clinical staff, liaising between services and service users, contacting services using the appropriate procedures/referral mechanisms.
Work closely with all relevant care agencies (primary care, secondary care, community services, Mental Health, Social Services, Ambulance Service, Voluntary Services, and other relevant service providers) to ensure a coordinated patient care plan, without requiring a further referral from the GP.
Act as the single point of contact for the PCN and establish systems and processes which will ensure a timely and appropriate response to queries from clinicians and other stakeholders.
Raise awareness of health promotion and NHS health checks in practices and with members of the local community.
Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact.
Refer to PCN social prescribing link workers where a patient is identified as potentially benefitting from this service.
Raise awareness of shared decision-making and decision support tools, assist people to be more prepared to have a shared decision-making conversation.
Reviewing and maintaining the clinical stock including protective personal equipment, stationary and medical equipment. Ordering as necessary.
Supporting the PCNs protected learning sessions with arranging speakers, caterers, issuing certificate of attendance and general on the day facilitation.
Communication:

Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
