Community Health & Social Care Hub Patient care Clinician

Hours22.5 hours -37.5hrs, negotiable

An exciting opportunity has arisen for the right candidate to join our clinical team within our community health & Social Care Hub.

Statement of Job Purpose

The post holder will have responsibility for providing high quality clinical advice and support to patients, carers and professionals in contact with the CH&SCH. Supporting administrative colleagues to ensure seamless, safe advice, support and care that is in line with the Maximising Independence agenda. Ensuring that assessments and care plans are recorded appropriately in line with Locala and NMC record keeping / HCPC Professional standards and that clinical advice, support and care always meets acceptable standards of patient safety and quality outcomes. The clinician will support administrative colleagues and respond to telephone enquires as appropriate assessing health care needs of patients, resolving some at source and ensuring the smooth transfer to others in accordance with clinical need, if appropriate. The post holder will be required to work with a high degree of autonomy and professional accountability. The primary aims of the role are to work closely with the admin colleagues in CH&SCH to maximise patients/ carers independence and prevent unnecessary referrals to locality/ expert teams.

Main duties of the job

To provide high quality evidence based clinical advice and support to patients and carers contacting the CH&SCH. To support the role of the Team Leader - Clinical. To support administrative colleagues in the CH&SCH. Be required to work in community to maintain clinical skills and competency and in the event to meet service need.
