Job overview

We are seeking experienced Band 5 Nurses with a background in community or acute care to join our innovative team. Our goal is to reduce emergency hospital admissions and reliance on acute hospital care through initiatives like integrated Community-based Frailty Hubs, expanded community services, and virtual wards. Virtual wards enable patients, who would typically be hospitalized, to receive acute care, remote monitoring, and treatment at home or their usual residence.

The post holder will support individuals with complex health issues and acute episodes, providing urgent responses within 1–2 hours or managing patients on virtual wards with stays of 1-14 days. Responsibilities include care planning, utilizing assessment tools, and escalating concerns to senior staff when necessary.

Main duties of the job

Monitoring patients, conducting holistic needs assessments using recognized tools and remote technology, recognizing patient deterioration, and escalating appropriately. Collaborating with professional leads, GP practices, and Primary Care Networks to develop valuable services tailored to local needs.

Person specification

Education/qualification

  • Diploma or Degree in Adult Nursing.
  • Current registration and evidence of CPD.
  • Understanding of clinical practice standards.
  • Diploma/Degree IV Cannulation Mentorship.
  • Willingness to undertake teaching and assessment.

Experience/Knowledge

  • Caseload management experience.
  • Minimum 2 years post-registration experience.
  • Understanding of lone and agile working.
  • Experience in acute and community settings.

Skills/Abilities

  • Core clinical skills: observations, manual handling, BLS.
  • Frailty assessments, including Clinical Frailty Score and NEWS.
  • Ability to complete comprehensive clinical assessments.
  • Ability to work independently in the community.
