Job summary

Hamilton Specialist Referrals in High Wycombe is seeking a Patient Pathway Nurse (Part Time) to join our RVN team. You will coordinate patient admissions and discharges and act as the central liaison with clients, clinicians and the administration team.

Based mainly in the wards, you’ll deliver high standards of care to complex referral cases, with a focus on excellent client service and effective communication throughout the patient journey.

Qualifications

• Registration as a Veterinary Nurse with the RCVS.
• Significant experience in clinical practice with an interest in client liaison and wards nursing.
• Strong communication skills and the ability to work effectively within multidisciplinary teams.
• A commitment to evidence-based practice, clinical governance, and continuous improvement.

Responsibilities

• Provide a consistent and client-focused experience by keeping clients informed throughout their pet’s journey, addressing questions, providing guidance on care plans.
• Oversee patient admissions and discharges, and manage inpatient care coordination whilst working clinically as a key part of our wards team. This will include I/V catheter placement, blood sampling, TPR & BP assessment, sedation/recoveries and nurse consults.
• Act as a pivotal link between clients, clinicians, and the Clinical Administration team to maintain clear and effective communication, supporting the best possible patient outcomes.
• Assist with audits and quality assurance within the hospital
