Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Progress Co-Ordinator at Bodmin Community Hospital. The role supports clinical staff in facilitating safe and timely discharge from hospital, working across teams and with Adult Social Care. You will need good IT and communication skills, with flexible hours including occasional weekends.

Qualifications

• NVQ 3 Business Administration or equivalent knowledge or experience.
• ECDL or similar IT literacy.
• Experience with health service systems is a plus.

Responsibilities

• Assist with the efficient day-to-day running of discharges, providing administrative and clerical support to the team, a 7-day service and flexible hours.
• Monitor patients' progress and report back on timing of interventions, equipment orders and ongoing care arrangements.
• Act as a link with other agencies to ensure smooth communication and expedited discharge to the chosen home environment.
