Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Part-time Primary Care Dementia Practitioner (Band 5) based in Bodmin. The role covers Dementia and Older People’s mental health, 22.3 hours per week across Monday–Friday, with a pro rata NHS salary of £32,073–£39,043 per year.

You will support people with dementia and their carers, liaise with GP practices, and help enable early diagnosis and access to services within the Cornwall primary care network.

Qualifications

• Current professional registration – NMC or HCPC.
• Experience with people living with dementia.
• Ability to work independently within community/primary care.
• Good communication with carers and families.

Responsibilities

• Provide specialist support to individuals living with dementia and their families, working with GP colleagues to ensure holistic care.
• Support through signposting, advice, education and practical support across care pathways.
• Identify individuals with dementia in the community not on GP registers to improve detection.
• Work in partnership with Complex Care and memory assessment teams to provide a comprehensive service.
