Job summary

Brookside Group Practice is recruiting a part-time practice pharmacist (17 hours/week) across its four surgeries serving about 31,000 patients. The role focuses on medication reviews, repeat prescriptions, and queries by telephone or clinic, with patient care in long-term conditions and medicines safety in care homes.

The post requires GPhC registration and at least two years' postgraduate pharmacy experience. Opportunities for clinical leadership and development are offered.

Qualifications

• GPhC registration is required.
• Two years' post-graduate pharmacy experience.

Responsibilities

• Provide medication reviews and queries to patients by telephone or in-clinic.
• Support the practice with the repeat prescription service.
• See patients with single or multiple conditions needing medicine optimisation.
• Provide pharmacist support to clinicians in care homes and attend as appropriate.
• Improve medicines safety with care home staff and optimise ordering/administration.
