Job summary

Gloucestershire Hospitals NHS Foundation Trust is seeking a Community Palliative Care Nurse Specialist (Band 6) for part-time 24 hours per week. The post involves working 9am-5pm over 3 weekdays, supporting patients with complex symptom management in community settings and coordinating care with GPs, district nurses and other colleagues.

This role requires current NMC registration, substantial post-registration experience in palliative care or related fields, and a patient-centred approach.

Qualifications

• Registered Nurse with current NMC registration.
• Significant post-registration experience, ideally within palliative care, oncology, community nursing or a related specialty.
• Excellent communication and interpersonal skills.

Responsibilities

• Assess and manage patients with complex palliative and end-of-life care needs.
• Provide specialist symptom management advice and support.
• Develop personalised care plans in partnership with patients and families.
• Support advance care planning and decision-making conversations.
• Educate and support healthcare professionals across the system.
• Contribute to multidisciplinary team meetings and care coordination.
