Epsom and St Helier University Hospitals NHS Trust is seeking a part-time Clinical Nurse Specialist for Heart Failure. You will join an expanding cardiology team and develop pathways to improve access to specialist care for heart failure patients.

You will work with the lead Heart Failure specialist nurse, the Cardiology Matron, and the lead Heart Failure Consultant to promote high-quality care across tertiary to primary care, including education and support for patients and carers.

Qualifications

• Experience in heart failure care or cardiology nursing is desirable.
• Interest in education, research, training and development.
• Ability to work with patients, relatives and carers in inpatient and outpatient settings.

Responsabilités

• Run and develop diagnostic and treatment pathways for heart failure patients.
• Support clinical staff in heart failure management plans and referrals.
• Promote high-quality care across the care continuum from tertiary to primary care.
• Engage with community heart failure teams and contribute to audits.
