Job summary

Home Group Limited in Yeovil is seeking a Health Coach to support customers in adopting healthy lifestyles. This permanent, part-time role offers £27,500 to £28,300 a year and the opportunity to work with a multidisciplinary team to empower individuals.

You will conduct assessments, develop personalised health plans, and deliver interventions to help clients reach their goals. A DBS check is required; travel to visits is expected with mileage paid between sites.

Qualifications

• Experience working in the community supporting people, ideally with their health.
• Knowledge of inequalities and barriers that impact health and wellbeing.
• Ability to motivate and support others, handle conflict and challenging behaviour.
• NVQ in Health and Social Care, Health Trainer and/or Coaching qualification preferred.
• Must have access to transport or vehicle insured for business use.

Responsibilities

• Coaches customers as part of a multi-disciplinary team to transform lives.
• Coaches customers with mental health issues to take control and ownership of their health.
• Carry out assessments and create individual health plans to enable healthy behaviours.
• Deliver interventions to help customers achieve health goals identified in their plan.
• Motivate others to adopt personalised healthy lifestyles.
