An exciting opportunity for Band 2 Support Workers to join a community-based multidisciplinary team at University Hospitals Coventry and Warwickshire NHS Trust. The role focuses on providing reablement support to patients, aiming to prevent avoidable hospital admissions and facilitate early discharges. This position offers part-time hours with a supportive development environment.
An exciting opportunity has arisen within the One Coventry Integrated Team for Band 2 Support Workers to join a community-based multidisciplinary team, providing reablement support for patients experiencing an acute deterioration in health and/or functional ability.
The aim of the service is to prevent and reduce avoidable hospital admissions and facilitate early discharges by supporting patients through reablement to maximise independence. The role includes identifying when independence is not achievable and signposting patients to appropriate long-term support.
Please note that a full valid UK driving license and access to a vehicle for business use are required for this role.

As a valued team member, your development will be supported through a robust induction, value-based appraisals, one-to-one meetings, regular in-service training, and supervision. You will work from a multi-professional base, offering opportunities for joint working with Advanced Clinical Practitioners, Nurses, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers, Support Workers, and other health and social care professionals.
Part-time hours are available, working a mixture of shifts:
Working up to 24 hours per week across 4 days out of 7.
