Job summary

Qualifications

• Experience in carrying out initial risk and needs assessments in respect of individual service users, either in the service user’s home or in other settings such as hospitals or residential/nursing homes.

Responsibilities

• Carry out supervision and support of care workers.
• Assist in planning realistic, effective care worker rotas with care coordinators.
• Review individual services and conduct risk assessments and renewal needs.
• Shadow and observe new care workers to ensure competence.
• Split time between community visits and office-based administrative tasks.
