Qualifications

• Full NHS Performer Number (if applicable)
• GDC certificate or registration
• UK Blood test results (Hep B, Hep C, HIV, TB) within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (last 12-36 months)
• Clinical references (2 referees, last 2 years)
• Degree certificate
• CPR/AED/Medical Emergencies cert (last 12 months)

Responsibilities

• Provide general dentistry in a mixed NHS/private practice.
