Kent and Medway Mental Health NHS Trust invites applications for a Multidisciplinary Pathway Worker in Canterbury. The role can be filled by Registered Mental Health Nurse, Occupational Therapist, Social Worker, or CYP-IAPT practitioner, delivering assessments, formulations, care planning and evidence-based interventions for CYP up to age 18.

You will work within CYPMHS pathways, collaborate with schools, GPs and social care, maintain records, with regular supervision and a part-time contract.

Qualifikationen

• Demonstrable skill in risk assessment and formulation.
• Experience delivering evidence-based therapies to children and young people.
• Strong communication and multi-agency working abilities.

Aufgaben

• Clinical Assessment & Formulation: Undertake comprehensive assessments of mental state, risk, safeguarding, and psychosocial needs to co-produce care plans.
• Evidence-Based Interventions: Deliver child-centred, family-focused therapies such as CBT, Systemic Family Practice, psychoeducation, and parent support.
• Outcome Monitoring: Use routine measures to track progress and review care plans.
• Multi-Agency Collaboration: Partner with schools, GPs, paediatric teams, social services, and third-sector organisations.
• Governance & Service Improvement: Participate in MDT meetings, clinical reviews, audits, safeguarding discussions, and service development.
• Team Support & Clinical Administration: Maintain accurate electronic records and support junior colleagues and students.
