Job summary

Bracknell Forest Council is recruiting a Community Support Worker on a part-time basis (18.5 hours per week) to deliver rehabilitation and care in residents' homes or settings. The role is on a fixed three-week rolling rota, with a requirement to have access to a car and participate in a multi-disciplinary team.

The package includes London Weighting and ECU, with a fixed annual leave structure and progression opportunities within the Intermediate Care Service.

Responsibilities

• Work as part of an integrated, multi-disciplinary team.
• Provide a planned programme of rehabilitation, care and support following an assessment.
• Monitor and record observations; maintain accurate records and review risk and outcomes.
• Undertake statutory and mandatory training and develop competencies in care and rehabilitation.
• Promote health, falls prevention, medication support, nutrition and healthy living to aid recovery.
