Job summary

JustOT is seeking a part-time Occupational Therapist to join our team. The role involves delivering OT services in clients' homes and community settings, focusing on independence and timely support. You will carry out assessments, develop tailored plans, and coordinate equipment and home adaptations.

The position requires flexibility for urgent visits and a weekly duty rota, with a Monday service cover. 3 days per week, 9:00–17:00 hours.

Responsibilities

• Deliver occupational therapy interventions in patients' homes and community settings, supporting independence and preventing hospital admissions.
• Conduct comprehensive assessments and develop tailored intervention plans for individuals with long-term complex needs.
• Support admission avoidance through timely and effective therapeutic input.
• Recommend and arrange assistive equipment and home adaptations, including processing Disabled Facilities Grants (DFGs).
• Conduct housing assessments and contribute to manual handling and reablement plans.
• Participate in a weekly Duty rota, responding to queries and undertaking urgent visits.
• Act as a Trusted Assessor to set up care packages based on assessed needs and complete reviews of existing care arrangements.
• Manage a varied caseload efficiently, demonstrating strong organisational and clinical reasoning skills.
