Job summary

Sussex Community NHS Foundation Trust is seeking a Heart Failure Specialist Nurse to join a well-established community team on a part-time basis (22.5 hours per week), across the High Weald, Lewes and Havens, with potential to work across the SCFT footprint.

The role focuses on case management of newly diagnosed and long-standing heart failure patients, aims to optimise guideline therapies, prevent admissions, and support palliative care when appropriate.

Qualifications

• Expert knowledge of heart failure management.

Responsibilities

• Provide heart failure services across the designated locality with advanced clinical assessment and medications management.
• Educate patients and provide psychological support to improve self-management.
• Collaborate with the acute HF service and attend weekly MDT.
• Support carers and signpost to appropriate services.
• Contribute to documentation and data collection for service evaluation.
• Assist in recruitment, appraisal, risk assessment and performance management.
• Provide clinical supervision and mentorship to junior colleagues.
