We have a great opportunity to work within the Community Parkinson's Specialist Service for either a Nurse or a AHP. This is a really exciting time to join our Multidisciplinary Team.

You will be working within the Department of Health, Nursing and Midwifery Council and NICE guidelines to support the Parkinson's Disease Nurse Specialist (PDNS) in the provision of professional expertise and evidence based clinical leadership for people with Parkinson's. The service will be provided across the North Hampshire.

The post holder will manage a clinical case load and will also provide education and training for health care professionals.

Main duties of the job

The successful candidate will work with the PDNS to facilitate and manage a caseload, identifying patients for admission to caseload, plus discharges and/or onward referrals as appropriate, according to need and priority. You will develop knowledge and expertise in the treatment used for the management of Parkinson's within the Trust, Professional and National policy frameworks. You will demonstrate ability to support patients in a range of settings, including domiciliary visits to a patient's home, nursing homes and Community Hospitals.

Person Specification
Qualifications
  • Current RN registration with the NMC
  • Able to demonstrate evidence of continuous professional development
  • Registered Nurse at Level One
  • Relevant Parkinson's qualification obtained at recognised centre or willingness to undertake training.
  • Valid driving licence.
  • Teaching qualification
  • Non Medical Prescribing Qualification.
  • Hold current registration as an Independent and Supplementary non-medical prescriber with the NMC.
Experience
  • Knowledge and understanding of the National Guidelines for governing care of people with Parkinson's.
  • Ability to demonstrate a commitment to multi-disciplinary team working.
  • Demonstrate the ability to work collaboratively with other agencies.
  • Willingness to demonstrate the potential for 'change management' approaches which extend existing practices
  • Exposure to, understanding of and competence in IT facilities and systems.
  • Significant experience of the care of people with Parkinson's.
  • In depth experience of the care of people with Parkinson's
  • Experience of caseload management.
  • Experience of service development.
  • Able to illustrate examples of service development initiated by the candidate.
Additional Criteria
  • Able to prioritise workload and manage capacity according to demand.
  • Ability to fulfil the travel requirements of the post. Current driving licence and access to vehicle for work.
