Job description
Overview

To be clinically and professionally responsible for Parkinson's patients throughout the pathway, providing and maintaining a high standard of care. To maintain and continually develop specialist knowledge and skills pertinent to the clinical area. To carry out home visits or review patients in nursing home and community hospital settings. To actively liaise with the multidisciplinary team and the wider organisation to ensure a seamless pathway of care for service users. Work autonomously, and take responsibility for the ongoing care and support of service users using a broad range of highly specialist complex advanced clinical nursing and decision making skills, which may include prescribing where appropriate. Contribute to ongoing service development which has a strong focus on audit, research and evidence based practice.

  • Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through ongoing self-education and attendance at relevant courses of study.
  • Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level.
  • Maintain own professional portfolio.
  • Contribute to the development of specialist nursing knowledge and skills to enhance and extend the practice of other nurses and health care professionals within the service.
  • Identify and address the educational needs of patients, families, carers and staff involved in the delivery of this service.
  • Contribute to the teaching and support of students from the multi professional team within relevant educational programmes.
  • Provide an appropriate learning environment, including mentorship, coaching and preceptorship.
  • Contribute to relevant educational programmes and in house study days.
  • Improve quality and develop practice: Parkinson's Specialist Nurses will be able to participate in ongoing service review and evaluation, including monitoring of patient experience.
  • Participate in research / audit to generate new evidence in area of speciality.
  • Evaluate clinical practice in relation to its evidence base and clinical effectiveness.
  • Identify areas of risk and poor quality, and address these through appropriate governance structures and forums.
  • Participate and actively seek peer review in practice.
  • Have responsibility to ensure annual appraisal, agree objectives which reflect local and national service needs and develop personal development plans.
  • Develop personal and professional knowledge through critical self-appraisal, clinical supervision and networking with other health care professionals locally, nationally and internationally.
  • Advocate and contribute to a working environment that promotes continuous learning and development, evidence based practice and succession planning, as well as identifying learning opportunities at local and national levels.
  • Have a high level of communication skills to enable the participation in the dissemination of practice through conferences and professional publications.
  • Ensure statutory and mandatory training updates, as well as role /speciality updates are completed and necessary records maintained.
General Conditions

The management of risk is the responsibility of everyone and will be achieved within a progressive, honest and open environment. The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

The Parkinson's Nurse is a highly experienced, knowledgeable and educated member of the Parkinson's Service and Lionel Cosin Day Hospital who is able to work autonomously making high level decisions to treat the health needs of patients with Parkinsons. The Parkinson's Nurse will be expected to visit patients at their homes, nursing homes or community hospital. Practice a high level of communication skills and organisational ability to sustain partnership with individuals, groups, managers, stakeholders and industry to ensure the high profile of the service is maintained.

Qualifications
  • First level degree (HEI level 5 or 6) in nursing
  • Demonstrable post graduate level working
  • Teaching / Assessment / Mentoring qualification
Desirable criteria
  • Relevant specialist post-graduate qualification in Neurosciences or equivalent clinical experience
  • Registered Non-medical prescriber, Knowledge of the key professional issues and NMC guidelines relating to professional practice
  • Evidence of being able to work across organisational boundaries
  • Demonstrates up to date knowledge and extensive expertise in the speciality
  • You must have appropriate UK professional registration
