Job description

A rare and exciting opportunity has arisen for a Band 7 Community Parkinson's Specialist Nurse to join our established community Parkinson's Nurse team at Livewell Southwest. We cover the Livewell footprint and offer a diverse caseload of patients, working in both city and rural settings, which informs our patient-centred approach.

This is a new, full-time role created through generous pump-priming funding from Parkinson's UK. The position is fully funded for 2 years, with the possibility of becoming permanent. It is a Fixed Term contract for 2 years and can be taken as a secondment with manager approval. The role is hybrid, allowing the successful candidate to work from home or the Cumberland Centre in Plymouth, conducting clinical reviews in person, via telephone, or virtual reviews across Plymouth, West Devon, and parts of South Hams.

Main Duties of the Job

We seek an enthusiastic, motivated, and passionate Nurse to deliver a patient-focused, evidence-based service, centering patients in their care pathways. The post holder will work autonomously, managing their own caseload, and will be supported by a wider Neurology and HCE clinical team in the Southwest. This role is suitable for general or mental health nurses, requiring the non-medical prescribing qualification V300 or willingness to undertake it.

Job Responsibilities

People Management

Manage staff engagement, workforce planning, appraisals, training, and performance management, with support from operational leads and service managers. Provide education to patients and staff on managing Parkinson's, supported by organizational policies. Identify practice development areas, support change initiatives, and serve as a role model and clinical advisor within the team. Conduct appraisals, mentorship, and supervision, and act as a role model demonstrating high standards of practice and conduct. Support colleagues in managing unpredictable workloads and assist with investigations of complaints and incidents. Promote team well-being and communicate effectively with the clinical team regarding treatment and prescribing updates. Collaborate with medical staff and service-users on clinical management plans and liaise with external agencies as needed.

Financial and Resource Management

Use resources efficiently, support workforce and budget planning, monitor expenditure, and support operational management with business cases and resource governance, including assessments for equipment and service eligibility.

Administrative Responsibilities

Organize own time effectively, maintain accurate and confidential records, and adhere to organizational policies. Use provided mobile and lone working devices responsibly. Report safety incidents and manage referral triage, working with operational and strategic leads to deliver the service.

Patient and Service User Care

Manage a caseload of approximately 700 patients with Parkinson's and related syndromes across Plymouth, West Devon, and South Hams. Deliver clinical and therapeutic interventions, including medication management and prescribing as an independent prescriber. Conduct holistic assessments, involving patients and carers in decision-making, and coordinate care with MDTs. Support complex case management to prevent hospital admissions and facilitate self-management. Liaise with healthcare and social care agencies, document assessments, and support patients through various stages of their disease, including end-of-life care. Use innovative care models and technology, such as the PKG watch, to enhance service delivery. Provide ongoing support and education to patients, carers, and staff, and contribute to clinical standards and service development.
