A primary care network in Hanley is seeking a dedicated Paramedic to provide high-quality patient care in allocated care homes. The role involves leading a diverse team to ensure timely health assessments, treatment, and adherence to care protocols. The ideal candidate will have experience collaborating with healthcare professionals and maintaining accurate health records, adhering to national guidelines and evidence-based practices.
Alarge focus of the role is to work as a Paramedic providing high quality andtimely patient centred care within the care homes allocated to supporting thedelivery of the Enhancing Health in Care Homes Network DES specification. Thepost holder will be responsible for the assessment, diagnosis, treatment andreferral of care home patients within their level of competence and agreedprotocols. This role will lead the widercare home team which includes: clinical pharmacists and pharmacy technicians, anANP, a healthcare assistant and administrative support. The teamcollaboratively deliver both acute and urgent care, long term conditionreviews, personalised care planning and general support to the care homes.
Collaborate with other members of the PCN including doctors, nurses and AHPs, accepting referrals and referring to them for specialist care.
Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local carepathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice
Work with patients in order to support compliance with and adherence to prescribed treatments
