Job description

We have a great opportunity for an experienced Advanced Care Nurse or Paramedic Practitioner to join our established and growing Duty Team. Working alongside the Duty Doctor and our existing ACP/ANPs, you will triage and treat patients with acute medical needs, working autonomously within your areas of qualification and experience and collaboratively on your ongoing training and development.

Main duties of the job

We are active in our local Primary Care Network and boast a diverse clinical team supported by Clinical Pharmacists, Pharmacy Technicians, Social Prescribing Link Workers, Health & Wellbeing Coaches, and First Contact Physiotherapists.

Hours vary between 08:00 and 18:30 on Tuesday and Wednesday, with a later clinic on Wednesday evening. Pay rates are based on transferable skills and experience.

We are an established and recently expanded GP Practice located in the heart of historic St Ives, Cambridgeshire. We specialize in research and training and support the development of our teams and colleagues. We host GP registrars, medical, and nursing students.

Job responsibilities

Clinical:

  • Work as part of a multidisciplinary team to deliver safe and high-quality care.
  • Perform clinical practice at an advanced level using expert skills, critical judgments, and knowledge to ensure appropriate and safe care.
  • Assess, diagnose, and treat patients requiring medical attention, including home visits, or refer to appropriate agencies.
  • Triaging and treating patients, performing specialist assessments, and making referrals to other healthcare team members as necessary.
  • Request and interpret diagnostic investigations within protocols, e.g., x-ray, blood tests, scans.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug treatments.
  • Prescribe and review medications appropriate to patient needs, following current evidence-based practice and protocols.
  • Coordinate care planning and delivery, involving patients and carers, and ensuring continuity of care.

Care Management:

Communicate confidential and sensitive information effectively. Provide specialized care advice, contribute to practice targets, monitor outcomes, and recommend changes.

Health Improvement:

Manage screening programs, promote health priorities, innovate practice, and support health promotion and disease prevention initiatives.

Administration and Professional Responsibilities:

  • Maintain accurate patient records and ensure clinical systems are up to date.
  • Participate in practice meetings, develop protocols, and maintain clinical areas.

Training and Personal Development:

  • Keep skills current through education, training, and development activities.
  • Participate in staff training, mentorship, and evaluation processes.

Confidentiality and Safety:

Adhere to data protection laws, maintain confidentiality, and ensure health and safety policies are followed, including infection control and safeguarding policies.
