A healthcare provider in Haywards Heath seeks a part-time Paramedic Practitioner to support urgent care services. The role includes assessment, diagnosis, and treatment of patients in various settings. Candidates should have HCPC registration, excellent communication skills, and relevant clinical experience. Due to a member of our team relocating, we have a part-time vacancy, 24 hrs per week, for a Paramedic Practitioner to support the daily on call team to provide an on the day urgent service for our patients. The role will involve the assessment, diagnosis, treatment orreferral of patients presenting with undifferentiated and undiagnosedconditions. This will be through telephone triage, electronic consultations andface-to-face consultations. The post holder will workwithin the Paramedic Practitioners level of competency. The PP will be required to respond to referrals and requests from various sources including but not limited to: patients, care homes, GPs, & NHS111
KEY RESPONSIBILITIES
To provideassessment, diagnosis and treatment at first point of contact by attendingto patients according to patients needs either in the Practice orpatients homes.
To support andcontinue the development of an on the day emergency and urgent patientservice in the Practice.
To accuratelytriage and prioritise patients, including in emergency situations,demonstrating the use of a variety of techniques to elicit the history ofan event/illness, including past medical and drug history.
To evaluateclinical information from examination and history taking, and initiateappropriate treatment and/or referral.
To undertake andinterpret diagnostics and take appropriate action to manage patient care,whilst distinguishing between abnormal and normal findings in order torelate them to a possible diagnosis. To use broad clinical guidelines(NICE etc) relating them to individual cases, providing assistance andfeedback to the organisation and others on how these should beinterpreted.
To signpostappropriately to other services in the identified healthcare pathway.
To refer patientsto appropriate care settings, within locally agreed healthcare pathways.
To discharge ortreat, as appropriate and within the scope of practice.
To maintainaccurate and systematic documentation of patient interactions and clinicalcare provided, including adverse events.
To advise patients on healthy lifestyles and health promotion.

KEY RESPONSIBILITIES
Clinical

To be competent onthe telephone, able to get clinical history and effectively triage patientsvia the telephone and online consultations.
To maintaincommunication with all healthcare professionals and outside agencies toensure seamless, patient-centred service provision.
To ensureeffective communication so that the patient receives the appropriate levelof care and wherever possible avoid unnecessary hospital admission.
To take an activerole in maintaining expert communication with all patients, carers andrelatives which meets their individual needs.
To deal withcomplaints in a calm and courteous manner, ensuring that, whereverpossible, complaints are dealt with efficiently and satisfactorily on alocal level.
To communicate effectively in demanding and challenging situations, where there may also be hostility.
Professional
To input to andextract information from the Practice Emis clinical IT system and otherclinical software used for patient care.
To work collaborativelywith other members of the health care team such as the District NursingTeam, Mental Health Services, staff in Care Homes, Ambulance Service,Secondary Care etc.
To ensure that thewellbeing of patients and staff is maintained in accordance with Practicepolicies.
Toundertake clinical audits in line with the organisations clinical audit plan,and to participate as required in equipment testing and other research.
Tocontribute to the analysis of information to inform future development of theservice.
Toparticipate and contribute to clinical meetings
Toidentify risks to service provision, and highlight them to the Clinical Lead.
Towork within the requirements of Data Protection legislation as it applies topatient records.
To maintaincurrent professional registration.
To act at alltimes in such a manner as to promote confidence and public trust and touphold the reputation of Newtons Practice and their own profession.
Organisational
To operate as aneffective member of the Practice team
To maintainaccurate and systematic documentation of patient interactions and clinicalcare provided, including any adverse events, child protection issues orother notifiable matters.
To provideinformation to inform the evaluation and efficiency of the role and forperformance management purposes as requested.
To engage in selfappraisal and utilise reflective practice to enhance and develop personaland organisational practice.
To assist inmarketing and promoting the role and organisation within the healthcommunity both locally and nationally.
Managerial
To ensure effectivetime management and prioritisation of workload.
To ensure thesecurity, safe storage and maintenance of any allocated equipment andgoods.
To ensure practiceis within good clinical governance and based on sound up-to- date clinicalevidence. Where appropriate, to identify and assess any risks associatedwith patient care or to oneself and communicate these risks to theappropriate clinical manager.
