Job description
  • Experience in an urgent care or primary care setting is desirable
  • Independent prescriber (preferred) or willingness to work towards it
  • Excellent clinical, communication, and decision-making skills
  • Ability to work independently and as part of a multi-disciplinary team
  • Confident and efficient using Microsoft Office and IT based patient clinical systems (EMIS is preferable)
  • Must have access to own vehicle and relevant up to date business insurance
  • Work across our 3 sites as required

We are looking for a motivated and caring Paramedic Practitioner to join our friendly GP practice team. This is an exciting opportunity to use your clinical skills in a hybrid general practice and patient home based environment. You'll be part of a close-knit, multidisciplinary team, helping us to provide safe, high quality care both in the surgery and out in the community. Main duties of the job
  • Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate
  • You will assess, diagnose, treat, refer or signpost patients who attend the surgery
  • Provide clinical assessment, diagnosis, treatment, and evaluation of patients
  • Work collaboratively with GPs, nurses, pharmacy Technician and admin team
  • Support the management of long-term conditions and care planning
  • Support and participate in shared learning across the practice

Job responsibilities
  • Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate
  • You will assess, diagnose, treat, refer or signpost patients who attend the surgery
  • Provide clinical assessment, diagnosis, treatment, and evaluation of patients
  • Work collaboratively with GPs, nurses, pharmacy Technician and admin team
  • Support the management of long-term conditions and care planning
  • Support and participate in shared learning across the practice
  • Assess patients presenting with acute illnessor for review of chronic disease conditions
  • Undertake home visits as required
  • Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting to other community or voluntary services
  • Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered.
  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
  • Maintain accurate healthrecords appropriate to the consultation, ensuring accurate completion ofall necessary documentation associated with patient health care andregistration with the practice.
  • Independent prescribing. We will considerapplications from Paramedic Practitioners who have not yet achieved this, they would be supported to do so in time.
Person Specification
Qualifications
  • HCPC registered paramedic with no current issues under investigation.
  • Educated to degree/diploma level in paramedicine or equivalent. Level 3 Safeguarding (children). Driving Licence held with business insurance
  • Independent prescribing
  • Completion of the FCP course (support for portfolio progression towards FCP/ACP can be provided in line with the HEE Roadmap to Practice)
Experience
  • Applicants must have current UK professional registration.
  • Experience of working in a GP Practice
  • Experience of working in primary care
  • Experience of EMIS
