A healthcare provider in Fareham is seeking a registered Paramedic to join their Complex Care Team. The role involves providing clinical management for frail patients, conducting assessments, and collaborating with multidisciplinary teams. Candidates should be registered paramedics with postgraduate qualifications and strong communication skills. The position is for 22.5 hours over 3 days and includes benefits like NHS pension scheme and employee assistance programs.
Employed by the Meon Health Practice (MHP), the post holder willbe a registered Paramedic with post-graduate qualifications relevant to theposition. Acting within their professional boundaries, the post holder willprovide care for patients presenting with undifferentiated conditions. Thiswill l include initial history taking, clinical assessment, diagnosis, treatment,and evaluation of care. They will demonstrate safe clinical decision-making andcare for patients within the MHP.
The post holder will demonstrate critical thinking in the clinicaldecision-making process.
They will work collaboratively within the Complex Care Team tomeet the needs of patients, supporting the delivery of policy and procedures.
They will work as an autonomous practitioner to provideclinical case management for frail patients with acute / chronic / palliativecare needs and at risk of further deterioration in health that may result in unplannedavoidable hospital admissions or unnecessary length of hospital stay.

Remote consulting and visiting MHP patients in CQC registered Carehomes (nursing / residential) and housebound patients registered with thenetwork practices to achieve acute visiting and assessment early in the day(typically before mid-day) to optimise patient outcomes.
The role will comprise twospecific disciplines:
1.Working within the MHP complex care team you will undertakeproactive consultations with Nursing / Residential home and housebound patientswithin MHP. You will provide advice to care home staff on patient care andwider health policy. This includes patient specific care plans and DNACPRdecisions.
2.Working alongside a centrally based duty GP you will deliver ahigh standard of patient care to patients who have been placed onto a centralGP triaged visiting list by practices within MHP
As a Primary Care ParamedicPractitionerwithin the complex care team, you will be using advancedautonomous clinical skills and a broad, in-depth theoretical knowledge base, toprovide patient care. This will include referring patients to hospital or othercommunity services. You will be working autonomously (in the patients home ora care home) with access to electronic patient records and available escalationto the complex care GP/ANP or duty GP for advice and support as required. Youwill work alongside Clinical Pharmacists from within MHP and the CCG to deliverthe appropriate medicines related care.
MHP delivers care to patients registered at either Whiteley, Jubilee or Highlands Practice in Fareham.
Our Organisation has a number of well established services and you will be working alongside GP's, ANP's, Practice Nurses, Pharmacists and Social Prescribers .

The post-holder is required to travel independently betweensites, patient homes and to attend meetings etc hosted by other agencies
Thepost-holder will have contact with bodily fluids, i.e. wound exudates, urine,blood etc whilst in clinical practice and must take precautions as requiredto minimise the risk of infection/contamination using PPE.
