Job summary

The Hospice of the Good Shepherd invites applications for a Social Worker to join our multidisciplinary team on a 6-month fixed-term maternity cover. The role supports palliative and end-of-life care for patients and their families, working three days per week with a pro-rated salary of £24,481 (FTE £40,803).

You will contribute to holistic care planning, advance care planning, safeguarding, and discharge planning, while showing empathy, resilience and discretion when interacting with patients,

Qualifications

  • Qualified Social Worker with appropriate professional registration.
  • Ability to work within a multidisciplinary hospice team.
  • Commitment to compassionate, person-centred care at end of life.

Responsibilities

  • Function as an integral member of the hospice multi-disciplinary team, contributing to holistic care planning and providing support that addresses the complex social, psychological and emotional needs of people approaching the end of life or living with a palliative diagnosis.
  • Use patient-identified goals and an understanding of individual social and psychological needs to contribute to multidisciplinary care and discharge planning.
  • Play a key role in initiating conversations and supporting the implementation of Advance Care Planning decisions.
  • Act as the Designated Safeguarding Lead, supporting the MDT in applying the principles and requirements of the Mental Capacity Act and Care Act, safeguarding patients' rights, autonomy and wellbeing, and delivering mandatory safeguarding training.
  • Demonstrate an understanding of the impact of grief and loss on patients, families and those important to them.
  • Demonstrate the ability to identify and respond to the emotional, psychological and cultural needs of patients and those important to them.
  • Work collaboratively as part of a wider integrated team, demonstrating strong leadership, communication and relationship-building skills.

Job description

£24,481 (FTE £40,803) 3 days per week. Fixed term contract - 6 months, Maternity Cover.

We have an exciting opportunity for a Social Worker to join our multi-disciplinary team at the Hospice of the Good Shepherd on a 6-month fixed-term contract to provide maternity cover.

You will be a key member of the multi-disciplinary team, dedicated to improving palliative and end-of-life care for patients and their families. The successful candidate will have a positive outlook, be emotionally resilient, and be able to work effectively with people experiencing significant distress. You will demonstrate the ability to work efficiently while recognising the importance of empathy, sensitivity, discretion and confidentiality when supporting patients, families, colleagues and members of the public.

The post holder will:
