Job summary

Marie Curie is recruiting a Band 6 Social Worker to join the Patient and Family Support Team across Liverpool Hospice, community settings and local hospitals.

You will provide social work and bereavement support to people with life-limiting illness and their families, advocating for care choices and coordinating services across care settings.

Qualifications

• Degree in Social Work with current registration with the relevant professional body.
• Significant post-qualifying experience supporting patients and families affected by life-limiting or terminal illness.
• Knowledge of palliative and end-of-life care, bereavement and loss.
• Knowledge of relevant social work legislation, safeguarding and deprivation of liberty safeguards.
• Experience of assessment, care management and working within a multidisciplinary team.
• Excellent communication skills and ability to work autonomously and collaboratively.
• Ability to organise workload, meet deadlines, and travel across settings.

Job Description

We are recruiting a Social Worker to join our Patient and Family Support Team in the North-West. Working across our Liverpool Hospice, community settings and local hospitals, you will provide responsive social work and bereavement support to people with life-limiting or terminal illness, their families and those important to them.

This is a varied and fast-paced role where no two days will be the same. Working alongside colleagues across the hospice, community and local hospitals, you will respond flexibly to changing needs while providing compassionate, person-centred support. You will advocate for patients and families, help people navigate complex decisions and services, and support safe, well-planned transitions of care.

Role: Band 6 Social Worker

Hours: 30 hours per week, across 4 or 5 days

Working pattern: Monday to Friday, 9am to 5pm

Location: Liverpool Hospice, community settings and local hospitals

Salary: Marie Curie Clinical Pay Scale, Band 6 £39,959 - £48,117 per annum (pro rata), plus a whole range of Marie Curie benefits.

Travel: Regular travel across the service area; a driving licence is essential

What you will be doing

• Complete holistic assessments and triage, agree person-centred outcomes, and review support as needs change.
• Contribute to discharge planning, liaising with hospice, community, hospital and external services so that people receive joined-up support.
• Provide psychological and emotional support, including sensitive conversations about death, dying, anticipatory grief and bereavement.
• Advocate for patients and families, signpost to specialist services and provide benefits advice where appropriate.
• Identify and respond to safeguarding concerns, applying relevant legislation and guidance, including deprivation of liberty safeguards.
• Participate in multidisciplinary meetings and maintain clear, accurate and timely records.
• Maintain your professional registration and contribute to learning, reflective practice, audit and service improvement.

Qualifications

You will be a compassionate, organized and self-motivated Social Worker who can build trust, hold complex conversations with sensitivity, and manage a varied caseload across different care settings.

• A degree in Social Work, or an equivalent recognised qualification, with current registration with the relevant professional body.
• Significant post-qualifying experience supporting patients and families affected by life-limiting or terminal illness.
• Knowledge and understanding of palliative and end-of-life care, bereavement and loss.
• Knowledge of relevant social work legislation, safeguarding and deprivation of liberty safeguards.
• Experience of assessment, care management and working within a multidisciplinary team.
• Excellent communication skills and confidence working both autonomously and collaboratively.
• Ability to organise your workload, meet deadlines and respond flexibly across hospice, community and hospital settings.
• A driving licence and the ability to travel throughout the local service area.
