A local healthcare provider in Basingstoke is seeking a Palliative Care RN Coordinator to join their Hospice at Home team. This role involves providing expert, compassionate nursing care to patients in their last days of life, as well as leading a dedicated team. Candidates should have a strong background in palliative care, excellent clinical judgement, and a commitment to patient-centered service. The position requires a full UK driving license and entails working flexible hours, including weekends.
The H@H coordinator RN is a pivot role within this new service;you will be required to use your experience and developed nursing clinicaljudgement & decision-making skills in assessing & triaging referralsinto the service and coordinating the workflow. Were looking for a highly motivated, passionate Registered Nurseto join our expanding Hospice at Home (H@H) Team at St. Michaels Hospice. Thisis a specialist nursing role, providing compassionate, expert care to patientsin the last weeks and days of their lives. This is an ideal opportunity for a nurse who wishes to specialisein palliative and end of life care, or who ideally already has experience inthis area. Someone who has a passion for making a difference to patients andtheir families alongside a love of the nursing role are some of the keyaspirations we are looking for in the successful candidate. Experience ofworking within the community setting would be an advantage.
This role involves working across a 2 or4 week roster period, which will include working alternate weekends. The shiftstimes are 07:30 - 17:00. A full UK driving licence and access to a vehicle areessential, as you will be travelling across the community within the hospicescatchment area.

Working collaboratively with the wider multi-disciplinary team, youll assess, plan, and provide holistic care that meets the physical, emotional, and spiritual needs of patients. Youll also play a key role in contributing to service improvement and promoting best practice in end-of-life care.

TheHospice at Home multi-professional Team (H@H) provide a 24/7 planned andreactive service to those deemed to be in their last 2 weeks of life whorequire specialist palliative care advice and support.
TheH@H nursing team specialise in providing palliative care symptom advice,holistic assessments and support to patients at the End of Life. The teamprovide clinical nursing interventions and support to enable the patient toremain at home, prevent hospital admissions and support discharge from hospitalto home.
TheH@H team also support patients known to St. Michaels to provide a communitypatient visiting service which includes patients known to the Living WellServices, SMH MND Service and the SMH Long-Term Neurological ConditionsService.
Thepostholder will be responsible for the daily coordination and operationaldelivery of the clinical workload of the H@H team, as well as reactivelysupporting the delivery of the RN clinical interventions when service demandrequires this.
Thepostholder will hold line manager responsibility within the H@H SeniorHealthcare Assistant team.
