The VNA, one of several delivery networks for the Care New England Health System (CNE) is seeking a full time Palliative Care Nurse Practitioner or Physician Assistant to join our collaborative team in Rhode Island. The VNA provides a broad spectrum of home health, hospice services for adults, and the terminally ill.

Position Highlights

  • Provide medical visits to patients with advanced illness in community homes, nursing homes and assisted living facilities
  • Conduct comprehensive History/Physicals and specific palliative care for malignant pain symptom management, advanced chronic disease symptom management, provide patient and caregiver education while supporting the patient’s provider care team
  • Daytime work schedule with autonomy with patient scheduling

  • 1+ years of relative experience preferred
