The Palliative Discharge Clinical Nurse supports rapid discharges for palliative patients, enabling end-of-life patients to achieve their preferred care and death settings. The role requires flexibility, empathy, and effective communication skills to support patients, families, and staff through complex and emotional situations. Collaboration with multidisciplinary teams and external providers is essential.
Ideal candidates should have significant experience as a registered nurse with an interest in palliative and EOL care. This is a fixed-term, part-time position (22.5 hours/week) for 6 months.

Main duties of the job
The nurse will provide support and advice to end-of-life patients, coordinate care across hospital and community settings, and facilitate timely discharges aligned with patient preferences. The role involves raising awareness of palliative services, managing referrals, and ensuring seamless care transfer.
The post aims to enhance palliative care delivery and support patients and carers with physical, psychological, social, spiritual, and bereavement needs.
