We are looking for a highly motivated and dynamic nurse to join our community specialist palliative care team on a permanent basis. We are a friendly, professional and supportive multi-disciplinary team, who set out to provide high quality palliative care throughout Northamptonshire. Our aim is to empower others and deliver expert advice and support to improve the quality of life and death for people with advanced progressive illness. You will have excellent access to clinical and psychological supervision, relevant training, peer support and appraisal to underpin and develop you in your role.
Main duties of the jobThe Palliative Care Clinical Nurse Specialistwill join an integrated Specialist Palliative Care team that works in the community across three bases in Northamptonshire. They will be required to influence patient care by participating:
The PCCNS will be aligned to a patient caseload, ensuring continuity of care in partnership with the integrated specialist palliative care team, GPs, acute trust partners and generalist community teams. There is an expectation to ensure care reflects the strategic priorities of NHFT; ensuring a flexible, responsive service that meets the local needs of people across Northamptonshire, whilst upholding Care Quality Commission requirements
This is a Band 7, part-time 26 hour permanent post you will have a nominated base at Cynthia Spencer Hospice; but there may be a requirement to work across the county.
Previous experience in palliative care and excellent communication skills are essential for this post. A non-medical prescribing qualification is also essential.
Car owner / driver and current driving license essential or access to a car (unless you have a disability as defined by the Equality Act 2010).
