The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing care, support and rehabilitation within a community setting to facilitate seamless discharge from hospital or prevent admission. The post holder will undertake delegated tasks following a care plan and report to the Team Lead. This role supports skill development to work effectively with this client group.

We seek dynamic, caring individuals to drive our Discharge to Assess services in Surrey Downs. Candidates should have excellent organisational and time management skills, a passion for holistic patient-centered care, and a proactive approach to improving service quality and fostering a 'one team' culture. Part-time shifts in the mornings are available.

Main duties of the job

Support the 'Home First' approach, enabling patients to be supported at home for assessments and care, reducing unnecessary hospital stays. The @home service provides rapid, short-term health and social care support across organisational boundaries, working closely with health and social care services to facilitate timely discharges and ongoing assessments. The service operates 7 days a week from 8 am to 8 pm, requiring flexible staffing.

This role will evolve with ongoing skills, knowledge, and competency development.

Detailed job description and main responsibilities
Main Responsibilities and Duties
  • Deliver clinical and therapeutic care as per care plan.
  • Perform patient assessments under supervision, following delegation from registered practitioners.
  • Recognise and refer to other professionals as needed.
  • Provide concise handovers to team leads and colleagues.
  • Act as a link worker in areas like tissue viability, infection control, or manual handling, providing feedback to the team.
  • Promote health education aligned with policies.
  • Support patient independence in daily activities, respecting cultural and personal preferences.
  • Be aware of the needs of dying patients.
  • Use risk assessment tools to ensure safe practice.
  • Undertake clinical skills such as vital signs, blood tests, ECG, medication administration, and personal care, against competencies.
  • Report adverse signs promptly.
  • Ensure clinical environments are prepared and safe.
  • Use IT systems for documentation and data collection.
  • Provide evidence-based rehabilitation and care.
  • Maintain effective communication with patients, families, and MDT members.
  • Keep clear, accurate records following professional standards.
  • Act professionally, respecting privacy and confidentiality.
