Manchester University NHS Foundation Trust is seeking a registered Healthcare professional to support the Paediatric Liaison Pathway within CAMHS Crisis Care. The role works with a multidisciplinary team to improve care for children and young people admitted to paediatric wards across the MFT footprint.

The post holder will typically work Monday to Friday, 9am-5pm, with potential involvement in Rapid Response and Home Treatment pathways. Band 6 level with ongoing pathway development.

Qualifikationen

• Proven experience working with children and young people with mental health needs.
• Current professional registration with relevant body.
• Ability to work within a multi-disciplinary team.

Aufgaben

• Conduct ward assessments and coordinate discharge planning.
• Deliver person-centred care and safety planning for CYP.
• Liaise with education, GP, and other agencies as required.
• Support development and delivery of the Paediatric Liaison Pathway.
• Flexibly support Rapid Response and Home Treatment pathways.
