Overview

Are you enthusiastic, compassionate, self-motivated and want to develop your skills in Children\'s Diabetes care? We are looking for a Diabetes Support Worker to join our team. This post is for someone with a passion for delivering joined-up, supportive care to children with diabetes and their families in community settings. The Diabetes Educator will work as part of a multidisciplinary team under the supervision and guidance of the site lead paediatric diabetes specialist nurse, providing high-quality, evidence-based care and support to people with diabetes within the paediatric service (ages 0-19).

The successful candidate will work with the specialist Children\'s Diabetes Team to develop and deliver a well-organised and responsive service to this patient group. Training opportunities are available to further develop skills relevant to the post, with patient safety at the heart of everything we do. We are looking for someone dedicated to patient-focused care with excellent communication skills who is willing to deliver high-quality care.

Responsibilities
  • Empower children and young people with diabetes and their families/carers to manage their condition effectively by delivering education and health promotion, with emphasis on patients who have difficulty engaging with their health needs.
  • Provide specialist education and teaching for other professionals across care sectors, including schools, with practical training in insulin administration and blood glucose testing for school and nursery staff.
  • Provide basic care planning for CYP in schools/educational settings.
  • Liaise with other agencies and community teams to provide coordinated packages of care.
  • Record findings in clinical records and patient notes according to Trust policy.
  • Report concerns or changes in a patient\'s psychological, physiological or social conditions.
  • Set up and support patients in downloading and sharing diabetes technology data to relevant digital platforms.
  • Plan and prioritise workload with the Paediatric Diabetes Specialist Nursing Team and carry out delegated tasks within own competencies.

Other Information
  • To work closely with the Paediatric Diabetes Specialist Nursing Team to plan and prioritise workload and carry out delegated tasks within the remit of the post holder\'s own sphere of responsibilities and competencies.
  • To undertake biometric data collection and perform review tasks in outpatient clinic settings.
  • Assist patients with downloading and sharing their diabetes data to relevant digital platforms.
  • Record all findings in clinical records per Trust policy.
  • Any concerns or changes in a patient\'s condition to be reported promptly.
