A leading healthcare provider in Norwich is seeking a dedicated Diabetes Support Worker to join the Children's Diabetes team. The successful candidate will work closely with families to empower children in managing diabetes care. We are looking for a Diabetes Support worker to join our team. This is a really exciting post that will be right for someone who has a passion for delivering joined-up, supportive care to children with Diabetes and their families in Community settings.
The successful candidate will work with the specialist Children's Diabetes Team to develop and deliver a well-organised and responsive service to this patient group. There will be access to all relevant training opportunities to further develop skills necessary for the post. Ensuring that patient safety is at the heart of everything we do. We are looking for someone who is dedicated to patient-focused care, with excellent communication skills, who is willing to deliver high-quality care.

The Diabetes Educator will work as part of a multidisciplinary team under the supervision and guidance of the site lead paediatric diabetes specialist nurse, and will provide high-quality, evidence-based care and support of people with diabetes within the paediatric diabetes service aged 0-19.
Main duties of this role would include empowering children and young people with diabetes and their families/carers to manage their condition effectively by delivering education and health promotion as appropriate, with emphasis on patients who are having difficulty engaging with their health needs.
To provide specialist education teaching for other professionals across all care sectors, including schools and other educational establishments as appropriate, practical training in insulin administration and blood glucose testing to school and nursery staff and to provide basic care planning for CYP in schools/ educational settings.
Liaising with other agencies across the wider community and with our own community teams to provide coordinated packages of care.
Please note that this advertised position, which is part of occupation code 6131, does not meet the UKVI eligibility requirements for a Skilled Worker Visa (this includes if you are already in a sponsored post and looking to change employer). As this is a part-time position, it does not meet the minimum salary requirements, meaning the Trust would not be able to issue a Certificate of Sponsorship for this role.
