Job summary

Frimley Health NHS Foundation Trust is seeking a Paediatric Diabetes Family Support Worker (Band 4) at Wexham Park Hospital. This Part-time role (18.75 hours/week) is permanent and offers an opportunity to support children and families living with diabetes within a dedicated paediatric diabetes multidisciplinary team.

You will provide practical, emotional and social support, helping families manage day-to-day diabetes, engage with healthcare services and build confidence for growing

Qualifications

• Foundation Degree equivalent to Diploma level.
• Continued professional development documented through Trust PDR/KSF/Supervision and personal portfolio.

Responsibilities

• Provide practical, emotional and social support to children, young people and their families.
• Build positive relationships with children, families and the multidisciplinary team.
• Help families understand information from the diabetes team and reinforce advice.
• Support day-to-day diabetes management within clinical guidance.
• Encourage confidence and independence in diabetes self-management.
