Frimley Health NHS Foundation Trust's paediatric diabetes team at Wexham Park Hospital are hiring a Paediatric Diabetes Family Support Worker (Band 4) 18.75 hours per week, permanent. Join a supportive multidisciplinary team helping children and families managing diabetes.

You will provide practical, emotional and social support, build trusted relationships, and help families engage with healthcare services, schools, and community resources.

Responsibilities

• Provide practical, emotional and social support to children, young people and their families.
• Build positive, trusting relationships with children, young people, families, and members of the multidisciplinary team.
• Help families understand information provided by the diabetes team and reinforce agreed advice in an accessible and age-appropriate way.
• Support families with the day-to-day management of diabetes, within the boundaries of the role and under appropriate clinical guidance.
• Encourage children and young people to develop confidence and independence in managing their diabetes as they become older.
• Identify barriers that may affect diabetes management, such as family circumstances, emotional wellbeing, school issues, financial difficulties, or difficulties engaging with services.
• Signpost or refer families to appropriate community, social care, education, psychological, and voluntary-sector services when required.
• Work collaboratively with schools and education staff to support children with diabetes and promote appropriate support within the school environment.
• Support families to attend appointments and engage with the paediatric diabetes service.
