The role involves supporting the day-to-day management of an integrated, evidence-based Community Paediatric Bladder and Bowel Service. Responsibilities include delivering patient care efficiently, coordinating service provision within Sunderland and South Tyneside, managing a caseload at Tier 2 level, and working collaboratively with paediatric colleagues to provide accessible, multidisciplinary continence services. The role aims to reduce inappropriate referrals to secondary care and respond effectively to children's physical, psychological, and social needs by working alongside secondary care, education, social services, and other relevant teams.
The position also focuses on transforming service delivery through enhanced collaboration across hospital and community settings, meeting the needs of children with continence issues, including those with learning difficulties and disabilities, and working within local policies and guidelines. Key tasks include comprehensive bladder/bowel assessments, care planning, holistic healthcare delivery, collaboration across various service tiers, and ensuring compliance with NICE, Department of Health, and NHS England guidelines. The role involves triaging referrals, managing NHS disposable containment products, providing specialist advice and training, developing communication networks, and supporting patient safety systems. Additionally, the role supports user involvement and the transition of care into adult services.
