Qualifications

• Role requires Band 6 Children’s Community Nurse responsibilities in a community setting.

Responsibilities

• Take responsibility and accountability for the nursing management and autonomous delivery of care to a caseload of children with chronic, complex, acute and palliative care needs within the community.
• Act as the key health professional for children and young people and act as their advocate and co-ordinators of their care in the community.
• Provide professional, high-quality patient care to children & families in the community.
• Provide training locally to enable children and young people to manage their own medical conditions or support others to minimise restrictions on the child’s activities.
• Assist the Team Leader in effective care planning, implementation and evaluation of care that meets professional and clinical standards/guidelines.
• Facilitate early discharge and effective aftercare within the community setting, preventing admission and readmission to hospital.
• Aspire to accommodate families’ wishes of where they would like to reside when a child or young person reaches end of life.
